    I think it is due to nonuniform mixing of concrete(not proper slump value through out) and improper compaction.

    Due to this the concrete not properly compacted,so bond stree will not properly developed and due to moisture contact with reinforcement bar corrosion genrate on the reinforcement bar and crack developed on the surface of the concrete.
