What is cause of Honeycombing and cavities in this column concrete

There is no slurry seen, aggregates only seen and reinforcement bars also seen with cavities.

What is wrong happen to cause this.

The major reason of honeycomb concrete is poor grading. In this columns it seems that the grading of coarse and fine aggregate is not proper resulting honeycomb concrete.

Due to this the concrete not properly compacted,so bond stree will not properly developed and due to moisture contact with reinforcement bar corrosion genrate on the reinforcement bar and crack developed on the surface of the concrete.

In this type of casting of column. Honeycombing reason in my purpose fine aggregate is low mix and shuttering work not properly and vibrator not used. Need the main three above reason to become properly then it work to be done strongly.
 
The root cause of the issue - Poor mix. There is not enough cement. QC needs to catch this before the mix is placed and not allow it to be placed. Vibration/consolidation is certainly a secondary issue, but, without having a homogeneous mix, vibrating won't fix it.
 
As per the Picture & even for honeycomb, there are many technical reasons.
1) Improper proportion of concrete mix.
2) Improper W\C ratio i.e. either more water or even less water than requirement as per design.
3) If only cover block is not there, than at surface bars may appear but inside concrete must be firm & compact.
4) Due to either less or even more vibration by needle or without needle, segregation may be there.
5) Due to improper shuttering, slurry may come out while compaction & vibration.
