Concrete failure - What are the reasons for the failure in concrete structure?

There might be various reasons for failure
1. Design failure
2. Load cannot be applied on concrete structure unless concrete receive 70 to 100% Strength.
3. Application of concrete, Concrete shall be poured within 45-60mins dispatch from the Batching plant.
4. Excess water content in concrete later stage water get evaporated and form a Cracks.
5. Embedded metal structure, Exposed portions shall be covered with Burlap.. Or else Due to the heat of Metal structure transfered to embedded parts and heat evaporated.. And form gaps on surface of structure.
6. Improper concrete cover and improper Formwork cause bulgeness
