All I seen a technicians place a Metal CONE adjustcent to the concrete.. And place a T-SQUARE On top and measure from Top end of CONE to TOP end of concrete sample.

its always from the top. the work ability of concrete will be good if it has a slump around 10-15cm.

In ASTM standard Slump should measure from the center if you get shear sump retest again until True slump

In BS we can measure slump ,which ever is the highest point
