Slab covering will be less, such abnormality results after few years.
Plastering may not have carried out to protect slab from fumes and gases.
Some pipes are visible in slab, water may be seeped from this pipes and corroded bars resulting expansion of bars and bottom portion have detached.

Due to less concrete thickness and slab cover is not provided properly ... Also there may be an design fault due to improper design the slab may sag...

Minimum bottom cover is not provided and over dia of reinforcement are used. Leakages due to electrical conduits improper jointing and proper compaction.
