What is the under design and over design of concrete, and how do you do the optimization of mix?

There is no such funda of doing under or over designing of concrete mixes.

Concrete mix designs are optimize to save every single penny that can be saved without compromising and taking risk of failure in concrete.

When you need a concrete of M 25 and its yielding to more than M50, then there is a scope for optimizing the mix by adjusting everything and saving cost(Mostly cement content is reduced).

There are another ways of optimizing mixes is like to use fly ash or ggbs to replace the cement in mix and use of high water reducing admixture.

there is nothing acceptable below the border line, i.e. - Characteristic strength. If you reach to this level or going below it, you need to look seriously into it, as failure in concrete will never happen due to its got less strength in your cube test. but due to it has got low strength in actual poured concrete. Occurrence of failure in concrete is not like, you did not got strength and still you completed a structure and nothing happened. it need a small change like lateral pressure and boom, you did that.

We should never play with concrete and we should never ever believe on funda's like, concrete will gain strength in lateral stage as we have added xyz into it (Some fools search internet for information and understand it in wrong way like use of flyash or ggbs delays the setting and helps in gaining strength after 28 days.) because everything that is non sense when it does not matches the trend which we had earlier when there was strength gain was not an issue.

so there is nothing like under designed, there are only 2 kinds which we may say, Optimized and un-optimized mixes.
