Spacer bars are basically provided to create sufficient gap , between more than one layers of longitudinal rebars if combinedly designed to be placed closely in either tension zone or in compressor zones , for the coarse aggregates of the concrete going to be placed. The size of the spacer hence , depends primarily on the maximum size of the coarse aggregates + some extra for better workability ( minimum 5mm as per standards).

You area absolutely right as per code. but problems comes when aggregate size is 45mm+5mm = 50mm and we have to provide cover block in place of spacer bars.
When aggregate size is 25mm and at that time spacing required is 30 mm, in that situation we have to put 32 or 38 mm bar. and it is not feasible that we maake new indent for bar which is not widely used in project.
still it is individual project head choice what he wants.
