i am not a surveyor but i did survey in my collage time.
Base on it i made a sample raw format survey field book (you need to format it to as you want to have its look).
there is an error of 2mm in surveying record, you need to normalize and do correction in reduce levels as per you learn in collage or referring books.
Reduce levels of foundation bottom and top are calculated without adjusting the error.
