Root cause of this bridge failure?

I am not a structural designer nor have any experience in bridge works.

From the above picture what i see is, the bridge pillars came out of the pile foundation cap or a foundation just like we press a candle in cake during birthday celebration.

Its either a poorly design pillars or the engineer did not executed the reinforcing work of that particular column as per the details given (such type of things mostly happens due to lack of supervision or letting thing to get executed by contractors workmen on their own)

I do not think that, reinforcement bars which are used are very bad quality else the whole bridge might had broke down long back before this event occurs.

An those columns / pillars look like thrown out of the cake like a candle whose purpose is ended (absolute indication of poor workmanship or poor design detailing)
