What is the reason for such errors

From column center to L/3 beam rods same dia on both top and bottom can be provided.

Column reinforcement same dia rod with minimum 300mm "L" and 55dia laping can be provided.

It cant even be treated as a mistake or poor supervision.

below the column starter, i see bricks are placed.

As i am not a structural designer cant say much on how this can be fix without demolishing column part below and correcting the reinforcement as the reinforcement is not only out but look way out from column face.

In normal conditions were rebar gets displaced due to any reason, it can be brought back to its location by joggling in ratio of 1 inch horizontal in height of 2 feet.

Sudden joggles makes bars to loose its strength, so never ever we should allow sharp joggle. it should be joggled always with ratio greater than 1 inch in 2 feet.
