Why structure fail

By looking at the picture it looks like.

1. at plinth level - there is a weak layer of concrete.

2. reinforcement bars are lapped at single level instead of staggering the laps in bars.

3. might have designed improperly or use a low quality materials other than specified by structural consultant.

Columns are literally got crush at plinth level due to super structure load (Mostly due to weaker concrete in the most prone cracking zone)

Let me point out these:
1. Poor workmanship and supervision
2. The columns were were improperly designed to carry such load it is.
3. Too much wind pressure, or weak soil bearing which was overlooked or any other natural calamity.
