what will be the rate of PQC per square foot or square meter including all costs.?

Max permissible flyash content as per MORT&H is 20% of cementious material. How can you take 25%

What is ssd
Saturated surface dry condition, it mean, the aggregates do have completely filled voids with water in inside structure and dry from the surface.

I did not find any limitations like this mentioned in MORTH nor anyone stopped me from doing such concrete.

Please provide the details of such limitation for concrete manufacturing and not for manufacturing of cement.
